    Monday – Friday | 8:00 AM – 5:00 PM
    Patients are seen by appointment only.

  • Appointment schedule times vary according to the type of service being provided. We will gladly reserve a time for your appointment; however, in consideration for others, we do require at least 24-hour notice for cancellation of any appointment. We do understand that there are circumstances that may prevent you from keeping your appointment. Appointments fill quickly; therefore providing us with as much notice as possible, may allow adequate time to offer the time slot to another person that needs an appointment. In the event of a late cancellation within 24-hours of the appointment or a failed/no-show appointment, fees will be applied to your account and are due immediately.

  • In an effort to better serve our patients, we have implemented an automated voice attendant to assist with your telephone requests. We make every effort to answer your telephone call promptly, however, when our staff is busy assisting patients, your call will be directed to the automated attendant, at which time you will select the option that best meets your need:

  • Appointment scheduling, rescheduling, or cancellations - 1

  • Medication refill requests or questions regarding medications - 2

  • New patients wanting information - 3

  • Current spravato/ketamine treatment patients - 4

  • Spravato inquiries - 5

  • PRESCRIPTION AND REFILL REQUESTS

    Prescriptions and requests for refills are handled during the regular business hours.
    To request a prescription or refill, please call the office at (337) 602-1462.

    -Date of your next appointment

    Your medication will be refilled as soon as your provider authorizes the refill. If you have questions about your medication, please leave your number and message and your call will be returned as soon as possible.
